    Download, fully complete, print, and sign one copy of the Brazil visa application form. 

    PLEASE NOTE: the application form is attached as a sample as well.

    Provide your SIGNED passport. Your passport must be signed on the appropriate line. (For US citizens the line is located on the page following your information page). The passport must be valid with an expiration date greater than six (6) months from the date of entry for the visa you are applying for. Passport must have sufficient blank pages (at least 2 pages) for any visa stamps. These pages must say VISAS at the top of the page.  

    If your passport does not meet the requirements, we can assist with renewing a US Passport or obtain a new US Passport from us prior to obtaining the visa.

    You must provide a passport photo. This passport photo must match the requirements of a standard US passport photo. They must be taken at an official passport photo provider (pharmacy, Costco, post office).  You can upload a digitial photo here for us to process.

    Photo Requirements:

    - Your head must face the camera directly with full face in view.

    - You must have a neutral facial expression or a natural smile, with both eyes open.

    - Taken in clothing normally worn on a daily basis

    - Taken in the last 6 months

    - Use a plain white or off-white background

    A copy of immigration status in the United States such as a resident card, work, or study permit.

    Copy of driver's license or state issued ID, or ORIGINAL major utility bill (Water, Gas, Electric, Sewage) for the most recent month, showing the applicant's name and current address. The address must not contain a PO Box. Address MUST match home address in your applicant profile.

    Copy of computer generated round trip tickets or travel itinerary showing round trip travel entering Brazil and exiting Brazil.  Preferably, the proof of departure documents should show your departure from the United States to Brazil and back.  Additionally, the Applicant's name must be on the travel document.

    Provide 3 most recent bank statements or proof of sufficient funds to be used during your travels. Applicant's name should appear on statements.

    A copy of hotel reservations or other lodgings for the duration of the trip. The applicant's name must be on the reservation.

    A background check/certificate of good conduct issued by INTERPOL.

    Notarized Police Clearance Certificate authenticated by the Department of State stating that applicant has no pending criminal proceedings.

    Notarized copy of applicant’s ordination certificate and/or diploma and school transcript for theological studies.

    Provide a Letter of Guarantee obtained at a "Cartório" by the religious or missionary organization in Brazil, guaranteeing the maintenance of the applicant and his/her family in Brazil and their repatriation at term.

    Charter and bylaws of the religious or missionary organization in Brazil; Minutes of Constitutions. A certified copy of the minutes of the Board of Directors General Assembly (Ata de Constituição) that created the institution and authorized its operations in Brazil; Statement of commitment. Signed statement of commitment that the applicant will not engage in any activity in Indian areas without the proper authorization by FUNAI - National Indian Foundation (notarized document plus one copy); Documents of Immediate Family Members. Marriage certificate, birth certificate, or other official dependency document for spouse/ children/dependents and other immediate family members, as applicable. This document must be legalized by the Consulate or Embassy of Brazil.

    A copy of a letter from your employer on business letterhead, with contact details, stating that a leave of absence has been granted, purpose and duration of the trip, and that you will be returning to your current job. If you are self-employed, include a copy of your business license and tax return. For students, please submit an official letter from your school indicating that you are in good standing and that you are registered for the upcoming semester. If you are retired please submit proof of your retirement fund.

3.1 Place the above documents in an Overnight Envelope

Enclose the documents from Section 1 (Gather Documents for your Brazil  Visa) and a print out of your order confirmation in an Overnight Envelope (Fedex, UPS, etc).
